Boarding Information Form

Our goal at Lassiter Animal Hospital is to provide the best available healthcare for your pet.  Please read carefully as you fill out the form below.  We are available at (770) 998-5100 to answer any questions.  Boarding rates are on a per pet basis.

There will be an additional $2 per day charge for any medication.

Additional Services Requested:

  • If the patient is admitted with fleas, we will require Capstar and a bath, or the purchase of a flea product.
  • Please complete the medical treatment options form if you have not done so previously. We will keep the treatment options form on file and refer to it for al future boarding. If your treatment option has changed from a previous visit, please inform the receptionists, so a new form can be completed.
  • In a life-threatening emergency situation, we will make every effort to contact you. However, if we are unable to reach you, we must provide emergency medical care at your expense.

    • We welcome the opportunity to care for your pet and hope that his/her stay here will be a happy time for you both.
